What documentation is needed to approve sick leave?

To approve sick leave, the following documentation may be required:

• For illness, accident, or pregnancy: medical documentation, such as a statement from a licensed health care provider, confirming the need to be absent.
• For adoption: a statement from the adoption agency showing the bonding period.
• For medical or dental appointments: time should only cover the appointment itself.
