Do I need to sign the form?

Yes, an employee’s signature is required to validate the request and indicate that the employee has read and understood the policies related to holding leave during FMLA. If an employee fails to sign the form, this may result in the delay or denial of the request to hold accrued leave.
 

If an employee is unable to make the election themselves because they are incapacitated, the form may be completed through an authorized Power of Attorney or representative.
