What documentation is required for Military Leave under FMLA?

Employees requesting FMLA Military Leave must provide specific documentation depending on the type of leave requested:
•    For a qualifying exigency, the employee must submit:
o    A copy of the covered military member’s active duty orders, and
o    A certification describing the facts related to the exigency (for example, travel details or contact information if meeting with a third party).
•    For rest and recuperation leave, the employee must provide:
o    A copy of the military member’s rest and recuperation orders, or
o    Other official military documentation showing the dates of the member’s leave.
•    For care of a covered service member with a serious injury or illness, the employee must submit:
o    A certification form completed by an authorized health care provider, or
o    A copy of an Invitational Travel Order (ITO) or Invitational Travel Authorization (ITA) issued by the military.
