What information can be requested in medical certification?

The FMLA Coordinator may ask only for the following details from the health care provider:
•    A statement of medical facts about the condition.
•    The start date, probable duration, and whether intermittent or reduced leave is needed.
•    Expected treatment schedule and number of treatments.
•    Whether the employee can perform essential job functions or needs to be absent for treatment.
•    For family care, whether the family member requires assistance or psychological comfort, and the estimated duration of care.
If the certification is incomplete or insufficient, the employer must give written notice of what’s missing and allow seven (7) calendar days to fix it.
Once a complete and sufficient certification is submitted, no additional information may be requested.
The FMLA Coordinator may contact the provider only to verify or clarify the certification’s authenticity.
