Who qualifies as a health care provider under FMLA?

A health care provider includes:
•    Licensed doctors of medicine or osteopathy
•    Podiatrists, dentists, clinical psychologists, optometrists, or chiropractors (for spinal adjustments shown by x-ray)
•    Nurse practitioners, nurse-midwives, clinical social workers, and physician assistants
•    Christian Science Practitioners listed with the First Church of Christ, Scientist (Boston, MA)
•    Any provider accepted by the State of Tennessee or Benefits Administration for benefit certification
•    Equivalent licensed health care providers practicing outside the United States
