How do I complete a student loan forgiveness form for a current or former TN State Government employee?

Current State Employees

For current TN State Government employees, please contact the HR office of the department where you work.  Please refer to https://www.tn.gov/hr/js1/human-resources-offices.html for a list of all HR offices.

 

Former State Employees

For former TN State Government employees, please contact the Division of Records Management at 615-741-5595.

For written verification of a former TN State Government employee, please fax the documentation to 615-401-7685 or email to DOHR.RecMgmt@tn.gov.
